URGENT SAME DAY APPOINTMENTS
A doctor assesses all requests for same day appointments based on clinical need. If you think you need a same day appointment/phone call please let us know by 12pm that day if possible.
An annual review is required for all patients with a chronic disease.
All patients with a chronic disease e.g. Diabetes, COPD/Asthma, Epilepsy etc. will be invited for an annual review. This invite may include a health questionnaire to be completed and returned prior to your review which will allow the clinician to review your current health status and help you get the most out of your appointment. You may also be asked to come for a blood test, have your BP & weight checked, or provide a urine sample before attending the clinic, in which case you will be informed beforehand.
Not all chronic diseases are reviewed by the Doctors. In fact, the majority are carried out by our Practice Nurses who are specially trained in chronic disease management. The nurse will review medication, undertake disease monitoring and in some cases a physical examination, and may offer lifestyle and dietary advice. The reception team will book your appointment with the most appropriate clinician.
