New Patients

New Patient Registration

If you live within our practice area and wish to register with our practice, please ask at reception for a New Patient Pack. 

You will be asked to complete a registration form and health questionnaire requesting details about your medical history and any medication you are taking.   You will need to bring with you your medical card or NHS number, and ID will be required for each registering person.  A list of acceptable identification documents will be included in your New Patient Pack.  If you take regular medication, please bring with you a copy of your repeat prescription slip.


Change in personal details - please notify us if you change your surname, address, telephone number, or e-mail address.


We only accept patients who live within our practice area. Should you move out of the area you will be required to register with a new practice near to your new home address.
 


TEMPORARY PATIENTS

You do not need to register unless you physically need to see a GP or Practice Nurse - e.g. for INR's, regular dressings etc, or have an acute problem which cannot wait until you can see your own GP.

  • You can access the Common Ailments Scheme provided by local pharmacies who offer free advice and medication
  • Emergency medication - you need to contact your own surgery or contact a pharmacy that offers CAS (see above)
  • If you are residing temporarily in the area, you cannot register as a temporary patient for the purposes of monthly prescriptions.  You must arrange these with your registered GP
